The InfoWell Patient Portal provides patients and clients with accurate and reliable information about their disease or condition. It was assembled by working groups of clinicians, including nurses, pharmacists, dietitians, physicians, rehab and community client service providers. Patients were also consulted and given the opportunity to provide feedback that was used for revisions to content and design.
InfoWell is a secure website with access to personal health information and care schedules as well as community resources, online tools, support initiatives and groups. Patients and clients are encouraged to increase their involvement and communication with their care providers to work together and reach consensus regarding information and goals.
