Patients and Families
InfoWell is available for patients and clients with Type 2 diabetes at the following seven SIMS partners: Bridgepoint Health, Central Community Care Access Centre, Providence Healthcare, St. John’s Rehab Hospital, Toronto Central Community Care Access Centre, Toronto Rehabilitation Institute, and University Health Network (UHN). InfoWell is also available at UHN's Princess Margaret Hospital, for Breast Cancer Survivorship patients enrolled in a research study, and Toronto General Hospital, for kidney disease (pre-dialysis) patients.
One-time registration is easy with InfoWell Patient Portal. You will have access to health information 24 hours a day, 7 days a week at home or anywhere there is a computer. Use the interactive tools to personalize and update your health information.
