Patient Portal

The InfoWell Patient Portal is a secure website created by SIMS in partnership with health care professionals and patients/clients. InfoWell provides access to health information and tools 24 hours a day, 7 days a week, helping patients/clients better manage their health and form closer relationships with their health care team. Use interactive tools to record a personalized list of medications including a refill reminder. Also find a journal to record daily thoughts, feelings and activities as well as a calendar to keep track of appointments.
